The team consists of these healthcare professionals:
- Provider

The main responsibility of your care team is to work for you.
Your care team reviews your conditions, goals, and progress and coordinates your care under the leadership and expertise of your Primary Care Provider. The team is also responsible for your ongoing care and can help explain why certain visits or procedures are recommended. Their overall goal is to help you to improve your health.
How can we help?
Your care team can help you:
- Work with other medical experts, if needed: If you need to see a specialist, your team will keep in touch with the specialist to make sure you get the care that you need.
- Track your health information using electronic records: Your medical team can track your health information using computers and electronic records. The team will have all of your records in one place.
- Access care when you need it: Your team has convenient office hours for appointments, including same day appointments, access to Urgent Care services, and after-hours advice from a healthcare provider who has access to your records for non-emergency medical needs.

What can you do to help?
- Be an active team player: Identify your health goals with your team and talk with them about your health questions. Share the successes and the challenges you have had with healthcare in the past. It’s important to tell your care team about any recent visits you have made to a hospital, emergency room, or urgent care center. Your care team will also ask for a list of all of the medications you are currently taking. You should always feel comfortable telling your team how you feel about the care you are receiving from them.
- Take care of your health: Follow the healthcare plan that you and your team have developed. Make sure that you understand how to follow the plan and set goals that you can reach. Once you begin to see results, you and your team can discuss adding new goals.
- Talk openly with your team: If you are having trouble sticking with your care plan, tell your team about it. If you feel your care plan is not working, you should communicate your concerns. Together, you and your team can make needed changes
